BRIEF CAUSE OF DEATH: Fractured skull, brain damage, laceration
of skull.
BRIEF MANNER OF DEATH: Gas explosion while commencing a welding
operation
inside the #1 chamber of a gasoline (pup) tanker.

RECOMMENDATIONS ISSUING FROM INQUIRY:
A purging technique be established by a governing body (for
example, the
Ministry of Labour) with consideration to the fact that water
(snow) and
gasoline form a mixture from which the gasoline can later release
to the
inside of the chamber.
Concerning the air sampler (sniffer), we recommend the following.
1. The unit be calibrated and certified by a qualified technical
laboratory
at least semi-annually.
2. A quick calibration procedure be established for the daily
use by the
operator.
3. The filament be replaced at least annually regardless of
frequency of
operation, and that this is to be done by the technical laboratory
and
certified by the laboratory.
4. One man be responsible for ensuring that the calibration,
daily testing
and maintenance be carried out and that he certify these facts
in
writing in a log book.
5. One man, as well as the person entering the chamber, verify
the readings
taken by the air sampler before entering the chamber.
6. The sample of air be taken both at the top of the tank and
inside the
valves (faucets) at the base of the tank.
7. A standard be established by a governing body (for example,
the Ministry
of Labour) to set a time period for retesting the air while
work is in
progress inside the chamber.
8. The Ministry of Labour inspect new companies within two weeks
of
commencing operations especially in the case of gas hazardous
operations.
