ACCIDENT DESCRIPTION:

Workers were attempting to thaw a frozen oil flow riser pipeline at an oil
storage tank. One worker stood on the top of the storage tank roof and ignited
a propane torch to apply heat to the pipeline. When the torch was ignited, an
explosion occurred which lifted the roof off the storage tank. The deceased
was propelled through the air, landing on the ground 92 feet (28m) from the
tank.
