ACCIDENT DESCRIPTION:
The deceased was inside a pressure trailer tanker applying a
coat of synthetic
rubber liner (anti-corrosive paint). The interior of the trailer
was
illuminated with a safety lamp using a quartz beam (500 watt).
A folded coat
hanger was wrapped around the light and the coat hanger's hook
was fastened to
the manhole flange. The light slipped off the flange, landed
on the floor of
the tanker and there was an explosion and fire. The deceased
sustained fatal
burns.
