ACCIDENT DESCRIPTION:

A worker was preparing to clean a 203.2mm pipeline which carried solution gas.
The pig had been placed in the pig chamber and into the throat of the 203.2mm
line. The chamber was then closed and the flare line was also closed. The
chamber was pressured and the main line valve was opened. The pig did not
move. The main line valve was then closed and the 25.4mm line for pressuring
up the chamber was also closed. The 101.6mm line from the chamber to flare was
opened to allow the chamber pressure to bleed to flare. When the chamber had
completed bleeding, the worker proceeded to open the chamber gate. A small
amount of gas escaped through the gate and dissipated. The deceased worker
then stepped in front of the chamber and proceeded to finish opening the gate.
At this time there was a loud bang and the pig flew out of the chamber,
striking the worker in the lower right abdomen. The worker was carried
approximately 7m by the force of the pig, striking against the piping system
behind him.
