ACCIDENT DESCRIPTION:

The deceased and another worker were cleaning a blockage in a sulphur seed
silo. For some unknown reason the deceased entered the silo through a manhole
opening on the top of the silo and began breaking up the sulphur with a
galvanized steel bar. The other worker had moved the transitional conveyor
away from the bottom of the silo, exposing the bottom opening. It is presumed
that the deceased struck the wall of the seed silo, which is also made of
steel, thus igniting the sulphur dust created by his breaking up of the plug.
There was a resulting explosion and fire. The helper, finding the deceased
worker with his shoulders and arms above the manhole, pulled him out and layed
him against the guardrail which was located around the tip of the silo.
Although mouth to mouth resucitation was commenced immediately, and continued
until the ambulance arrived, the other workers were unable to revive the
deceased.
