ACCIDENT DESCRIPTION:
While in the process of flushing an eight inch (203mm) pipeline
between valve
blocks, a cold cut was required to expedite the insertion of
a pig. Hazardous
vapours escaped at the cut, impregnating the air with combustible
material.
The combustible material was ignited causing an explosion and
fire, killing one
worker and injuring several workers.
