ACCIDENT DESCRIPTION:
Four workers were involved in sweeping oil from the 5F5 heater
in the process
area. Two workers had proceeded to the top platform (catwalk)
of the 5F5 to
close two valves on the snuffing steam line. The deceased worker
indicated to
a worker on the pipe rack near ground level to open valves on
a crossover
between the snuffing steam line and the charge line. The valves
were opened
and a vapor cloud was noticed around the worker on the platform
who was now
indicating he wished the valves closed. At this moment an explosion
and fire
engulfed the worker on the platform. The worker was fatally
injured and the
other worker found his way to ground.
