ACCIDENT DESCRIPTION:
A customer entered a retail dealership yard and proceeded to
back around a
parked transport van with his 1/2 ton truck. The customer was
attempting to
park his 1/2 ton truck by the stairway leading to the oxygen/acteylene
cylinder
storage shed. The customer backed his truck into an unprotected
bulk 4546l
(1000 gal.) propane storage tank knocking it off its 200mm thick
concrete pads.
The feed line from the tank broke off, releasing propane liquid
and gas which
filled the area. A worker entered the shop area connected to
the site office
15 minutes after the incident to turn off the furnace pilot
lights. He was
killed shortly after entering the building when the propane
fumes ignited and
caused an explosion.
