A worker was fatally injured while grinding a weldolet on the
lid of a metal
tool box. The tool box contained an undetermined amount of acetylene
or oxygen
acetylene combination. The sparks from the grinding operation
ignited the
explosive atmosphere, propelling the tool box lid up causing
the grinder to
strike the worker's head.
