ACCIDENT DESCRIPTION:
Five workers were in the process of changing filters on an inlet
gas filter
separator. After isolating the separator by closing off inlet
gas lines and
opening lines to flare, a safety valve was opened on the separator
which
indicated pressure and liquid in the separator. A second attempt
to open it
indicated a negative pressure reading from within the separator.
The workers
started to remove the yoke which held the head on the separator
when unexpected
gas pressure from within the separator blew the head of the
separator open. An
explosion and fire followed, injuring all five workers. One
was fatally
injured.
