ACCIDENT DESCRIPTION:
Crews were conducting
a hot oil stimulation of a gas producing well. A pressure
relief valve set at 2200 PSI was rigged into the annulus port of
the wellhead and connected through a chicksan to 54.9m of
50.8mm I.D. used production tubing through a reducing hose
fitting and flex hose to an underground waste tank. Midway
through the stimulation program the pressure relief valve
unloaded. Gas pressure caused the relief tubing to separate
at the 18.288m joint with force and jet action which whipped
the tubing around the wellhead, fatally injuring one
worker.
