electrocution. BRIEF MANNER OF DEATH: Broken housing of
coupling allowed improper pin contact causing 347 volts of
electricity to pass through body. ACCIDENT

RECOMMENDATIONS ISSUING FROM
INQUIRY:
1. Manufacturers consider design changes to the
male and female sockets of four different shaped pins and
relative pin holders, in addition to present safety
devices.
2. Manufacturers include safety instructions with
all individual shipments of electrical equipment.
3. The
Ontario Industrial Accident Prevention Association
acquaint companies with accident prevention rules and
regulations, particularly in regard to the use of electrical
equipment and the education of employers, employees and members
of the public in safety methods by the use of an advertising
campaign to include instruction in cardiac pulmonary
resuscitation procedures. All companies be encouraged to
hold regular safety inspections and meetings supported by
written instructions.
4. Companies be encouraged to
implement the use of ground fault interruptors where high
voltage is involved.
5. Canada Bread Co. be commended on
the prompt compliance with safety recommendations made by the
Ontario Industrial Safety Branch of the Ministry of
Labour.
