ACCIDENT DESCRIPTION:
Since there were no
eye witnesses, it is assumed that while checking the level of
water in a fibreglass storage tank (21' 6" (6.5 m) high and 11' 6"
(3.3 m) in diameter), the worker was partially overcome by H2S
gas. He fell to the ground, where he succumbed to injuries
sustained in the fall.
