ACCIDENT INFORMATION:

DATE OF ACCIDENT : 1979-08-21
PLACE OF ACCIDENT: Inco Mine, Shabandowan
BRIEF CAUSE OF DEATH: Suffocation due to compression of chest and
inhalation of stomach contents.
BRIEF MANNER OF DEATH: The timber supporting the boom and bucket slipped
causing the deceased to be pinned and consequently suffocated.

RECOMMENDATIONS ISSUING FROM INQUIRY:

1. Only timber or steel supports of a length to stand vertical should be
used to support boom and bucket scoops in the mines.

2. Steel wheel shocks be mandatory standard equipment.

3. We endorse the procedure for supporting the scoop tram and bucket
assembly proposed as outlined by Inco's November 9, 1979 memorandum.

4. Stepped up safety program for maintenance department with regular
meetings.

5. On-the-job demonstrations of all safety procedures with the reason for
their importance.

6. Uniform safety procedures in all mines in the province of Ontario.

7. Mandatory first aid program.
