ACCIDENT
DESCRIPTION:
The deceased went from the dog-house to check
around the rig as his shift started. He was found by a fellow
worker lying face down in a section of the mud-tank. It is
presumed that he had removed a piece of metal grating (used
as a cat-walk) in order to observe the fluid level and fell
into the tank.
