ACCIDENT DESCRIPTION:
When the
well effluent (oil and water) flowed over the blowout preventers,
one worker went to the blowout preventer pipe ram control and
collapsed while turning the handle. The foreman picked up the
breathing apparatus nearby, returned to assist the first
worker, and was also overcome. A third worker came to help
rescue the first and was also overcome. The crew pulled
the workers out of the gas area and managed to revive the
third, but the first worker and the foreman
died.
