ACCIDENT
DESCRIPTION:
The worker was overcome by escaping gas while
attempting to thaw the meter lines in the meter house. The
upstream valve was not closed when the worker disconnected the
meter line from the valve manifold. Methanol was then
poured into this line and fed down to the orifice where the
thawing process took place. When the restriction was removed in
the orifice, the gas flowed up the upstream meter line through
the valve manifold and out the disconnected fitting to the
atmosphere at a rate of 170 P.S.I.G., asphyxiating the
worker.
