ACCIDENT DESCRIPTION:
The deceased
was checking for leaks on an H2S cooler unit. He was told to
wear his air mask and to watch for water to come from a check
valve in a manhole 4 feet below the work level. He was found
lying at the bottom of the manhole without his air mask on. The
mask was on the top of the cooler unit, next to the manhole
opening, and appeared to be in good working
condition.
