ACCIDENT
DESCRIPTION:
The deceased was fatally injured when he
entered a 8' x 8' by 8' wooden shack in which a break in a gas
line was releasing natural gas under 500 P.S.I. The line was
broken by a D8 caterpillar with a ripper, just outside the
wooden shack. He was found by fellow workers laying face down
inside the shack.
