inhalation (lack of oxygen) SOURCE OF INJURY: hydrogen

ACCIDENT DESCRIPTION:
The workers
were pumping oil out of a leaking 500 barrel crude oil storage
tank in order to repair it. When the level of the oil had been
lowered to approximately 2 to 3 inches above the flat tank
floor, one worker entered the tank with a squeegee to help the
pump clean up the remaining oil. Immediately after entering the
tank this worker was overcome with H2S and the deceased entered
to rescue him. He was also overcome. A third worker was able
to rescue the first worker but not the
second.
