ACCIDENT DESCRIPTION:
A vacuum truck was
being prepared to pick up a load. The main suction/discharge
connection on the vacuum tank's rear bulk head
was malfunctioning. It is presumed the valve was frozen in the
partially open position. Of their own volition, workers chose
to use a propane open flamed torch to free the valve. Flame
propagation through the partially open valve ignited an
explosive mixture in the vacuum tank. The ignition blew off
the vacuum tank's rear door. The door struck two workers; it's
unsure what struck a third. All three were fatally
injured.
