ACCIDENT
DESCRIPTION:
Two workers were overcome by H2S while
attempting to remove a "hydrate plug" from a 75 mm fibreglass
pipeline. The accident occurred at the installation of a new
satellite battery. The "hydrate plug" moved in the line and
knocked a temporary fitting called a "dresser sleeve" off the
open end of the pipe, releasing H2S gas. One worker, without
wearing respiratory protective equipment, entered the
excavation, where the open end of the pipe was located, and was
overcome by H2S. Another worker, who was not wearing
respiratory protective equipment, was also overcome by H2S when
he entered the excavation to rescue the downed
worker.
