ACCIDENT DESCRIPTION:

The worker was cleaning a vessel in a sulfinol plant. After lunch break, the
workers returned to the work area to continue with the cleaning process. The
deceased entered the vessel from underneath the west end and emerged again
moments later. He went to the east end of the vessel and climbed the scaffold
to enter the vessel by the manway. The vessel was divided and there was a
small compartment in the east end which, apparently, had not been cleaned.
A worker saw him climb the scaffold, but did not see him enter the vessel. The
worker apparently entered into the vessel before the worker assigned to be
safety guard had returned from his lunch break. After some time, he was missed
by the crew. He was discovered lying in the east end compartment of the
vessel, in a pool of sludge. The rescue and subsequent resuscitation attempt
was unsuccessful.
