ACCIDENT DESCRIPTION:

The deceased was required, as part of his daily duties, to gauge the sour crude
oil storage tanks. The external (automatic) gauging system was broken, so the
deceased had to physically gauge the storage tank from the ladder, using a
steel tape with a plumb-bob attached, through the thief hatch on top of the
tank. When he opened the thief hatch (200mm in diameter) on top of the tank,
he was overcome by H2S gas fumes and fell 6.08m to the ground. The H2S content
of this well is 18-22%, and worker was not wearing respiratory protective
equipment at the time, although it was available in his truck. The worker's
body was discovered lying on the ground at the base of the ladder by a local
farmer when he went to investigate why the truck was still at the well site
with the lights on.
