ACCIDENT
DESCRIPTION:
The worker was welding a stairway from the top
of one gasoline storage tank down to another horizontal tank.
The worker was lying on top of the middle tank of a line of
three vertical tanks running north to south. A horizontal tank
was on the west side of the tank line. The tanks which were 6m in
height by 3.6m in diameter previously held hydrocarbon
products. During the welding operation the centre tank the
worker was working on exploded, throwing him to the ground
approximately 57.3m away from the tank on the west side of the
site. The centre tank which had not been cleaned, contained
sediment material about 152mm - 203mm deep containing volative
hydrocarbons.
