ACCIDENT DESCRIPTION:

Two workers were sinking a 0.75m diameter hole approximately 9m deep. They had
progressed to approximately 7m when an obstruction was encountered at the
bottom of the hole. One worker used the tail chain of the sand line (winch
line) to form a stirrup around his foot and was lowered to the bottom of the
hole with a shovel. After several minutes of silence from the bottom of the
hole, another worker went to the edge of the hole to investigate. The second
worker slid down the same sand line after attempting to revive the first
worker, he could be heard coughing or gasping by the foreman. When the foreman
realized that the two men were in trouble, he told a cat skinner, also working
on site, that there was gas in the hole and for no one to go in.
