ACCIDENT DESCRIPTION:
The worker
drove his loaded truck to an unloading dock. He was seen
connecting the unloading hose to the dock facilities. He was
later found unconscious on the catwalk on top of the truck
tank. There were 3 hatches opened in the top of the tank that
contained crude oil (oil containing H2S gas) and the
deceased was overcome by
H2S.
