ACCIDENT DESCRIPTION:
Worker was
engaged in measurement of the liquid level in a 4.9m oil
storage tank. An estimated 10 minutes following an unclear
radio communication with the worker, a second worker,
dispatched to the scene, found the deceased lying inverted on
the stairway leading to the tank walkway, in an unconscious
state. (Note: possible inhalation of a fine aerosal of liquid
hydrocarbon droplets)
